What are the options for completing enrollment forms?

We offer three convenient options for filling out enrollment forms. You can choose to complete the necessary paperwork online through our secure portal, download and print the forms to bring with you to your first appointment, or fill out the paperwork in the office upon arrival. Our office staff is available to assist you in navigating these options. If you opt to complete the forms in the office, please arrive 30 minutes early to ensure sufficient time before your scheduled appointment.

How can I make the most of my first appointment?

To make the most of your first visit, we recommend filling out the new patient paperwork at home and bringing it with you. If you haven't done so, arrive 30 minutes early to complete the paperwork in the office. Wear comfortable clothes for a wide range of movements and bring health and insurance details, as well as any relevant medical devices. If you have a referral, please bring that along as well.

How long does a typical appointment last?

Depending on the nature of your treatment, appointments can last approximately 30 minutes to one hour.

What should I bring to my first appointment?

Bring your health and insurance details, and any braces, crutches, or splints you use. If you have a referral, remember to bring that along as well.

Is insurance required for treatment?

No, insurance is not required. While it's beneficial, we accept everyone and will work with you to ensure you receive the care you need. If you opt for a payment plan, you can provide payment directly at the clinic.

How does the billing process work?

We handle everything for you. We verify your benefits, collect the estimated amount owed during your visit, and submit a claim to your insurance company. Processing by the insurance company may take up to 60 days. After processing, we review the claim and inform you of any balance or refund. Monthly statements for balances are sent out, and refunds are processed after completing your care and reviewing all claims.

Will my insurance cover physical therapy?

In many cases, yes. We will verify your benefits, and during your first visit, we explain them thoroughly. This complimentary service ensures transparency about your benefits. If you have specific questions, consult our insurance list or contact us for further clarification.

Can I request a referral to IPT from my doctor?
